Muskoka Community Health Link programs are part of a plan to improve the health and well-being of people living in Muskoka by ensuring we are providing the right care, at the right time, in the right place.
Muskoka's Community Health Link is focused on:
- Increasing Access to Primary Care through Muskoka Community Health Hubs
- Improving System Navigation through Coordinated Care Planning
- Reducing Alternative Level of Care needs through Community Capacity Building
The Muskoka Community Health Link is a partnership between the District Municipality of Muskoka, the Algonquin and Cottage Country Family Health Teams, the North Muskoka Nurse Practitioner-Led Clinic, the North Simcoe Muskoka Community Care Access Centre, Muskoka Algonquin Healthcare, Canadian Mental Health Association, Muskoka- Parry Sound Branch and other community agencies.
