Algonquin Family Health Team Geriatric Care

The Ministry of Health and Long Term care initiated the Family Health Teams as a new approach to providing primary care services.  With over 23 family doctors, nurse practitioners, a dietitian, two pharmacists, and mental health workers, the Algonquin Family Health Team (AFHT) provides care in the community, at seven family physician offices, the AFHT Building (Huntsville, ON), and the AFHT Wellness Hub at the Summit Centre.        
 
The Algonquin Family Health Team works towards ensuring high quality comprehensive and continual care through the coordination of a primary care team.  They also work as an innovative and responsive team with the patient as central member of the team approach.  There objective is to excel at providing patient education and patient advocacy to promote illness prevention and holistic care, as well as foster an environment in which to provide education to future health care professionals .
 
AFHT is able to offer a multi-faceted complement of staff providing services such as; nursing care in your physicians’ offices, dietician consultations, pharmacist consultations, mental health counseling, geriatric care services, palliative care services, cardiac rehabilitation programming.  Asthma, COPD and smoking cessation programming, and lastly; health promotion programming.
 
One of the programs central to the Algonquin Family Health Team is a program providing Geriatric Care.  At AFHT, the Geriatric Care team provides comprehensive geriatric assessment and assists seniors with complex health problems.  They also assist families/ caregivers in making decisions related to seniors’ care in an effort to maximize independent function and quality of life.
 
Geriatric care is responsive to patients who may be experiencing health management issues during the again process.  These issues can include depression, dementia, confusion, delirium, memory problems.  Recent functional decline, risk to self, falls, caregiver difficulty, polypharmacy, and other issues.
 
The Geriatric Care program is an outreach program.  Patients receive the care they need where they need it – in their home/residence or in the hospital.  Each member of our team is dedicated to the provision of care to the elderly. The team includes Registered Nurses, a Care of the Elderly Physician, a Clinical Pharmacist, and specialized Client Care Coordinators from CCAC. 
 
The goal of the Geriatric Care program is to maximize independent function and quality of life for seniors with complex health problems and their families/.caregivers.
 
The Geriatric Care team provides a number of supports to increase the functionality of each senior, some of these include disease management, family/caregiver support, information and advice, liaison with others, practical support and emotional support.
