Simcoe Muskoka Integrated Fall Strategy



Simcoe Muskoka Integrated Fall Strategy is a community partnership dedicated to preventing falls in older adults living in Simcoe County and the District of Muskoka.
            
Simcoe Muskoka Integrated Fall Strategy aims to reduce the number of fall-related injuries in older adults within Simcoe Muskoka by raising awareness of the impact falls have on the lives of older adults and their families, and providing opportunities for falls prevention education to the public and health care professionals.
            
The Coalition has hosted an annual educational event for health care providers in October 2008 and 2009.  As well as created an electronic inventory of falls prevention programs, services and resources for older adults and health care providers within Simcoe County and the District of Muskoka - launched October 14, 2009.  Lastly, the Simcoe Muskoka Integrated Fall Strategy has increased access to in-home and community activity programs through the SMART Program.  Services are provided by VON Simcoe County Branch, and funded by aging at home strategy, North Simcoe Muskoka LHIN
