Home At Last

Home At Last is a FREE SERVICE providing worry-free patient transition from hospital to home. Often, patients are ready to depart from the hospital, but have no transportation and / or support when they arrive at home. 
 
This service is available to seniors 65+ who need assistance when being released from participating hospitals in North Simcoe and Muskoka area. The hospital discharge planner will review eligibility with the patient prior to discharge.
 
The discharge planner or alternative hospital staff will arrange for Home At Last staff to drive you home, make sure you get settled in safely, and see that you receive any other services or assistance required to remain at home.
 
To help make this transition easier for patients and their families, Home At Last offers a number of services, including:
 
  • Helping patients leave the hospital and become settled at home.
  • Making the transition from the hospital to home easy and comfortable.
  • Providing a trained and caring Home At Last attendant, who will go home with the patient and stay until they are settled.
  • Providing reliable and comfortable transportation.
  • Picks up groceries, prescriptions, and medical supplies / equipment if required.
  • Performs light housekeeping, laundry, meal preparation and assists with personal care upon arrival at home.

What do We Want You to Know?
 
  1. We place you first. Giving you excellent care and service is our top priority.
  2. You WILL NOT be alone.
  3. A Home At Last attendant will stay with you at your home for a pre-arranged amount of time or until your family arrives to be sure that you are settled and have food and medication.
  4. The next day, a coordinator will call you to see how you are doing. S/he will also see that you get any other service you need to keep safe and healthy at home.
What Does the Patient Need to Do?
 
Patients are responsible for the items on the following checklist:
 
  • Be organized, dressed, and ready to go home at the pre-determined discharge time (from hospital).
  • Have appropriate clothing and footwear.
  • Make sure to take all personal belongings home from hospital.
  • Make sure to have a house key.
  • Have enough money for groceries, medications or medical supplies and / or equipment.
  • Understand medication instructions.
  • Make sure driveway, sidewalk, steps to house are cleared of snow and ice for safe access.
How Much Does it Cost?
 
  • The cost is FREE to the patient. North Simcoe Muskoka Local Health and Integration Network (The Ministry of Long-Term Care) covers all costs.
  • Patients pay for their own groceries, prescriptions, medical equipment, and additional support services.
